Hoss Sports Therapy is committed to protecting your privacy and legal rights when dealing with all patient’s personal information.

This privacy notice explains why we collect personal information about you (or anyone you have provided us with information about e.g. your child), how we use it and keep it secure.

Under the GDPR act (2018), we are required to advise our patient(s) on our Data Protection Policy. As part of the Patient Record, Hoss Sports Therapy is required to retain information for the purpose of consultation for treatment, recording subsequent treatments, and for use by third party medical practitioners only, at the request of the patient, in writing.

In this policy “personal information” means any personal data provided to us by you, or on your behalf, for the purpose of providing our services to you.

Upon completion of the GDPR form prior to the Initial Consultation, all paper files and information therein may be electronically scanned and stored on computer file for as long as the patient remains a patient of the practice, and thereafter for a period of 7 years. Alternatively, paper records will be retained for the same period.

All information provided will be treated as confidential and will not be given to any other person(s), organisation(s) without the written consent of the patient concerned. Information will be held both manually and electronically in files accessible only by staff of the clinic who are directly involved in the data entry, and processing of patient records.

We will always comply with the General Data Protection Regulation (GDPR) when dealing with your personal information. Further details on GDPR can be found at www.ico.gov.uk. For the purposes of GDPR we are the “data controller” of all personal information held about you.
